By signing this declaration, I confirm that all information provided to Leadcare Recruitment is true, complete, and accurate to the best of my knowledge. I understand that knowingly providing false, inaccurate, or misleading information may affect my registration, placements, or continued engagement.

I understand that my personal data will be processed and stored securely in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. Personal data will be retained only for as long as necessary to satisfy legal, regulatory, contractual, and governance obligations.

Personal data is processed on the lawful bases of contractual necessity, legal obligation, and legitimate interests, as applicable. I understand that certain processing activities required for regulatory, compliance, and safeguarding purposes may not rely solely on consent.

I confirm that I have the legal right to work in the United Kingdom. I understand that Leadcare Recruitment is required to verify and periodically re-verify my identity, eligibility, and right-to-work status in accordance with applicable legislation and regulatory standards.

I agree to provide valid documentation upon request and to notify Leadcare Recruitment immediately of any changes to my identity, immigration, visa, or right-to-work status.

I acknowledge that Leadcare Recruitment will process my personal data for legitimate business, regulatory, and compliance purposes, including but not limited to:

• Registration, compliance, and placement activities
• Verification of identity, qualifications, training, employment history, and professional registration
• Assessment of fitness to work, including Occupational Health processes where required
• Criminal record, safeguarding, and background checks (including AccessNI / DBS where applicable)
• Fraud prevention, audit, and regulatory compliance activities
• Disclosure of relevant information to authorised third parties where required for placements, governance, audit, or legal obligations (including clients, NHS organisations, professional regulators, and auditors)

I understand that relevant compliance and governance information may be shared with authorised parties where necessary to fulfil legal, contractual, regulatory, or safeguarding obligations.

I agree to comply with all applicable professional, ethical, regulatory, and legal requirements relevant to my role, and to adhere to all applicable professional standards, codes of conduct, and scope-of-practice requirements.

I agree to inform Leadcare Recruitment immediately if:

• My professional registration status changes
• My GMC registration, licence to practise, or revalidation status changes
• I become subject to investigation, restriction, suspension, undertakings, or conditions imposed by any regulator, employer, or authority
• I am charged, cautioned, or convicted of any criminal offence
• My right-to-work or immigration status changes
• My professional indemnity / medical defence cover changes, lapses, becomes restricted, or contains exclusions affecting my practice
• Any circumstances arise that may affect my fitness, suitability, or safe ability to practise

I confirm that I have disclosed my full scope of clinical practice and agree to notify Leadcare Recruitment of any material changes to my scope of work.

I confirm that I have disclosed any current or pending investigations, restrictions, undertakings, or conditions imposed by any regulator, licensing body, employer, or contracting authority.

I confirm that I have disclosed any relevant health matters through the appropriate Occupational Health process. To the best of my knowledge, I am not aware of any undisclosed condition that would impair my safe professional practice.

I understand that failure to disclose relevant information, or failure to notify material changes, may affect my registration, placements, or continued engagement with Leadcare Recruitment.
